For over a decade, XLHealth has been a leader in the development of programs and businesses that seek to improve healthcare for Medicare beneficiaries with special needs, such as those with chronic illnesses.

Chronic illnesses such as diabetes and heart failure place a heavy burden on the elderly — nearly 80 percent of all Medicare beneficiaries have at least one chronic health condition. In addition to the daily challenges of managing a chronic illness, beneficiaries with chronic health conditions account for a majority of healthcare spending. The costliest five percent of Medicare beneficiaries account for about half of all Medicare spending each year. Among this top five percent, approximately 47% have congestive heart failure and approximately 35% have diabetes.1

And our state-of-the-art information technology allows us to assess, stratify, and analyze patient care needs and outcomes—preventing avoidable and expensive disease complications and hospitalizations.
